ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
Alcohol must not be displayed in the premises
Alcohol must not be sold or supplied from the premises other than by means of hand delivery to a pre-booked address, with bona fide orders of food, where the cost of the food delivered exceeds £20 in value
Alcohol will not be sold, supplied or delivered within the vicinity of the premises
Alcohol must be sold and supplied in the manufacturers sealed container
A written policy must be in place to prevent the sale or supply of alcohol to persons under 18 years of age, the policy shall require any person making a delivery of alcohol to accept one of the following forms of identification before completing the delivery:
1 A recognised proof of age card accredited under the British Retail Consortiums Proof of Age Standards Scheme (PASS)
2 British Photo Driving Licence
3 Passport
4 Any other identification approved and accredited in writing by Lincoln Police Licensing Office
All point of sale staff must undergo training in the above policy with a record kept of the date of training, signed by the member of staff and the trainer, each entry shall be retained for a period of 24 months from the date of completion, this record shall be made available for inspection by Lincolnshire Police officers, WLDC Licensing Officers or other parties acting on their behalf
A written record must be kept of all alcohol orders delivered to include the time and date of order, the person making the order, method of payment, the delivery address and person receiving delivery and whether proof of age was requested and what means of identification was supplied, the name of the person making the delivery and signed by them and endorsed by the DPS
A secure lockable unit approved by Lincolnshire Police must be inside the delivery vehicle to discreetly transport alcohol out of public view at all times
