ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
Prevention of Crime & Disorder
CCTV shall be installed at the premises. The number and locations of the cameras/monitors and recording equipment to be agreed with the police.
Recordings shall be installed in a safe environment for a period of 30 days and made available to an authorised officer of the licensing authority or police.
The system shall be maintained and images stored in accordance with the Data Protection Act and any other relevant legislation relating to the use of the systems.
Any door staff as deemed necessary by a management risk assessment, or as agreed in conjunction with Lincolnshire police for any major pre arranged/special advertised events shall be licensed by the Security Industry Authority (SIA) in accordance with the Private Security Industry Act 2001.
Persons who are attempting to buy alcohol who appear to be under the age of 21 years shall be required to produce proof of age by way of police agreed proof of age scheme specifically: - Portman Prove It, a photo driving licence, passport or Citizen Card, or any other future agreed scheme.
Persons under the age of 18 shall be accompanied and supervised by an adult and shall not be on the premises after 7.00pm.
Seating to be provided for 170 persons.
Public Safety
On occasions identified by a management risk assessment or as agreed in conjunction with Lincolnshire Police for any major pre arranged/special advertised event, the following conditions shall apply;
A record shall be kept on the premises by the Designated Premises Supervisor of every person employed on the premises as a door supervisor. The record shall contain the following details:-
(i) name, address;
(ii) date of birth;
(iii) his/her Security Industry Authority licence number.
That record shall be retained for a period of 12 months and be available for inspection on demand to any police officer, Licensing Authority officer or representative of the Security Industry Authority.
A log book shall be maintained at the premises, in which shall be recorded the following details:-
(i) the door supervisor's name;
(ii) his/her Security Industry Authority licence number;
(iii) the time and date he/she starts and finishes duty;
(iv) each entry shall be signed by the door supervisor.
That log book shall be available for inspection on demand by an Authorised Officer of the Council, the Security Industry Authority or a Police Constable and retained for a period of 12 months.
Door supervisors to be positioned at each entrance/exit (other than fire exits) to ensure that no customer leaves with any bottles or glasses.
In order to comply with the venue's capacity limit a SIA registered security team must be employed to control entry to the premises.
Door supervisors registered with the SIA will remain outside the premises until at least 30 minutes after the last customer has left the premises.
The Licence holder (and/or their respective agents) will liaise with each of the relevant authorities to ensure that all appropriate inspections are carried out to the satisfaction of each of the respective authorities before the premises commence trading to the public.
The Licence holder will ensure compliance with all building regulations requirements prior to trading
The Licence holder will comply with the reasonable requirements of the Fire Officer applicable at the time of completion of the works.
the Licence holder will provide a fire risk assessment prior to the commencement of trading.
Prevention of Public Nuisance
To comply with the reasonable requirements of the environmental health officer, to include matters relating to acoustic measures, noxious smells and refuse.
Notices asking patrons to leave quietly shall be displayed at the premises.
Protection of Children form Harm
Persons under the age of 18 shall be accompanied and supervised by an adult and shall not be on the premises after 7.00pm.
Persons under the age of 18 will not be permitted to play the amusement with prizes machines.
