ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
General
A copy of Club rules shall be provided to the Police and Licensing Authority on grant of licence and any subsequent amendment to the said club rules.
There shall be no teenage birthday parties or functions.
Prevention of Crime & Disorder
Where the premises or any part of the them are provided for the purposes of any function/event where it can reasonably be expected that there will be a predominance of non-members in attendance, there shall be provided door supervisors to such a number as the management of the premises consider are sufficient to control the entry of persons to the premises, and for the keeping of order in the premises.
When they are used for the provision of a licensable activity a record/log book shall be kept on the premises by the Designated Premises Supervisor (DPS) of every person employed on the premises as a door supervisor, the record shall contain the following details:
The door supervisors name, his/her Security Industry Authority (SIA) Licence Number
Time and date he/she commenced and concluded their duty, signed by the door supervisor
The DPS or other authorised person shall also endorse each entry as having checked the authenticity of the individual door supervisor
The record shall be kept for inspection on demand by an Authorised Officer of the Council, the SIA or a Police Constable.
The CCTV system in place shall be maintained in working order and operated at the premises, the system shall be used to record during all hours when licensable activities are being conducted and the quality of the images recorded by the CCTV system shall be such that they are of practical value in the promotion of the licensing objectives.
The images recorded by the CCTV system shall be retained in unedited form for a period of not less than 28 days and must be endorsed with the accurate relevant time and date.
The recordings shall be provided to officers of Lincolnshire Police on request and Officers of the Lincolnshire Police shall be permitted access to the system at any reasonable time.
During all hours that the premises are open or licensable activities are permitted, a member of staff will at all times be on the premises and capable of operating the system in its entirety.
Protection of Children from Harm
There shall be in place for the premises a written policy to prevent the sale or supply of alcohol to persons under 18 years of age. The policy shall require any person who appears to be under the age of 21 years to produce one of the following forms of identification:
A recognised proof of age card accredited under the British Consortiums Proof of Age Standards Scheme (PASS)
Photo Driving Licence
Passport
Any other means of identification approved and accredited in writing by Lincoln Police Licensing Office
All point of sale staff shall undergo training in the above policy with a record kept of the date of training, signed by the member of staff and the trainer, each entry shall be kept for 24 months from date of completion. This record shall be made available for inspection by Lincolnshire Police Officers or other parties acting on their behalf.
A notice or notices shall be displayed in and at the entrance to the premises where they can be clearly seen and read and shall indicate:
That it is unlawful for persons under 18 to purchase alcohol or for any person to purchase alcohol on behalf of a person under 18 years of age
That proof of age may be requested at the premises and those accepted means for proof of age listed above
