CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
All licensable activities taking part on the premises shall be organised directly by the premises licence holder only
No third party bookings shall take place
All staff shall be given training in their responsibilities linked to the licensing objectives, led by the designated premises supervisor or any other personal licence holder designated on their behalf.
An incident book shall be kept at the premises in which details of crime and/or disorder relating
to the premises shall be recorded, the incident book shall include the following details:
a) Time, date and location of incident
b) Nature of the incident
c) Names, addresses and contact details of persons involved
d) Results of the incident
e) Action taken to prevent further such incidents
f) Each entry shall be signed by the DPS or other responsible person employed at the premises
and so authorised by the DPS
The incident book shall be made available to Police and Licensing Authority upon request and
shall be retained for a period of 24 months from date of completion
When door supervisors are working at the premises there shall be such a number as the Licence Holder considers sufficient to control the entry of persons to the premises and for the keeping of order in the premises, and the following conditions shall apply;
A record/log book shall be kept on the premises by the Designated Premises Supervisor of every person employed on the premises as a door supervisor.
The record shall contain the following details:-
The door supervisor's name;
His/Her Security Industry Authority Licence Number
The time and date he/she commenced and concluded their duty
The door supervisors shall sign each entry
The Designated Premises Supervisor or other authorised person shall also endorse each entry as having checked the authenticity of the individual door supervisor.
That record shall be available for inspection on demand by an Authorised Officer of the Council, the Security Industry Authority or a Police Constable. Each entry shall be retained for a period of 12 months from date of completion.
The Premises Licence Holder shall give 6 months written notice to Lincolnshire Police Licensing Office, Lincolnshire Police Events Planning Officer, the Event Planning Manager for Lincolnshire Emergency Planning and the Fire Safety Officer for Lincolnshire Fire and Rescue, of any event, where it is intended or can be reasonably expected, that more than 499 persons will attend the event.
This notice will enable a decision as to the necessity for a Safety Advisory Group for the event.
On any occasion when music is provided, there should be on site a senior member of staff who will be nominated to monitor noise levels and respond to any complaints received.
All external doors and windows shall be kept closed when regulated entertainment is being provided except in the event of an emergency.
There shall be notices placed at all exits from the premises and in a place where they can be seen and easily read by the public, requiring customers to leave the premises quietly.
There shall be in place at the premises a written policy to prevent the sale or supply of alcohol to persons under 18 years of age;
a) That policy shall require any person who appears to be under the age of 25 to produce one of the following forms of identification;
i. Full UK Driving Licence
ii. Passport
iii. A recognised proof of age card accredited under the Proof of Age Standards Scheme (PASS)
iv. Any other form of identification approved for use by the Chief Officer of Police for Lincolnshire.
b) All point of sale staff shall undergo training in the above policy;
i. With a record kept of the date of the training;
ii. Signed by the member of staff and the trainer;
iii. Each entry shall be retained for a period of 24 months from the date of completion;
iv. This record shall be made available for inspection by Lincolnshire Police Officers or parties acting on their behalf.
A notice or notices shall be displayed in and at the entrance to the premises where alcohol is being supplied that can be clearly seen and read and shall indicate:
a) That it is unlawful for persons under 18 to purchase alcohol or for any person to purchase alcohol on behalf of a person under 18 years of age
b) That the Challenge 25 scheme is in place
c) That proof of age may be requested at the premises
d) The acceptable means for proof of age shall be;
i. Full UK Driving Licence
ii. Passport
iii. A recognised proof of age card accredited under the Proof of Age Standards Scheme (PASS)
iv. Any other form of identification approved for use by the Chief Officer of Police for Lincolnshire.
Anyone serving alcohol on the premises shall be at least 18 years of age.
No persons under the age of 16 shall remain on the premises after 2100 hours when licensable activities are taking place.
