ANNEX 2 - CONDITIONS CONSISTENT WITH THE OPERATING SCHEDULE
The licence holder shall:
a) Provide written notice to the following, regarding any event to which this licence applies where it can be reasonably expected that more than 499 persons will attend the event:
Lincolnshire Police Licensing Department
Lincolnshire Police Event Planning Department
Fire Safety Officer, Lincolnshire Fire and Rescue
Lincolnshire County Council Emergency Planning Manager
West Lindsey District Licensing Department
b) Provide the aforementioned written notice no less than 3 calendar months before the date upon which an event shall commence.
c) Comply with all recommendations from the officers at condition 1(a) in relation to events to which this licence applies.
For the purpose of conditions 1(a) and 1(b), written notice will provide the following information:
a) The nature of the event.
b) The expected attendance of the event.
c) The date and time of the event.
When door supervisors are working at the premises to control the entry of persons to the premises and for the keeping of order in the premises, the following conditions shall apply:
A record/log book shall be kept on the premises by the Premises Licence Holder (PLH) of every person employed on the premises as a door supervisor
The record shall contain the following details:-
The door supervisors name, his/her Security Industry Authority (SIA) Licence Number, the time and date he/she commenced and concluded their duty, the door supervisors shall sign each entry, the PLH or authorised person shall also endorse each entry as having checked the authenticity of the individual door supervisor.
That record shall be available for inspection on demand by an authorised officer of the Council, the SIA or a Police Constable. Each entry shall be retained for a period of 12 months from date of completion.
Each door supervisors licence number will be verified by the PLH or their agent using the SIA website, on the occasion of each door supervisors initial employment at the premises. This verification check will be recorded and signed appropriately by the PLH or their agent.
